Building Bridges: Creating Sustainability in the Transitional Health Clinic for Refugees Through Planned Transitions to Community Providers

The Transitional Health Clinic for Refugees in Halifax, NS serves as a transitional Health Home for newly arrived refugees and claimants.  This presentation will focus on the model and evaluation plan.

Presented by Graeme Kohler (Health Services Manager, Primary Health Care, Nova Scotia Health Authority) and Ashley Sharpe (Registered Nurse, Transitional Health Clinic for Refugees), this seminar will present on the Transitional Health Clinic for Refugees.  The clinic model mirrors the settlement process by providing primary care for refugees during their first few years in Halifax.  Once families are determined to have an adequate level of readiness for transition, they are matched with a local primary care provider.  Research is currently underway on this innovative model.
